Things to Remember Before You Begin
Convenient Form Filling:
Both forms are fillable PDFs, which can be filled out directly in your web browser.
After completing the forms, make sure to save the edited versions for easy submission or emailing.
If you prefer, you can fill out the forms by hand.
Submission Options:
Email the completed forms to lakesideartistsacademy@gmail.com.
Bring the completed forms in person on the first day of class. (Please let us know in advance if you’re choosing this option.)
Liability Waiver:
On the first day of class, a Liability Waiver must be signed to finalize your registration.
STEP 1
Step 1: Download the Application Forms
Fill out the Academy Application Form
This form collects key details about parents/guardians and other family information, such as allergies.
Each student will need their own completed student sections.
This includes a list of check boxes for selecting the specific classes the student will take.
STEP 2
Step 2: Fill Out the Forms
Fill out the forms directly in your web browser, or print them out to complete by hand if you plan to submit them in person.
Ensure all fields are completed, including the checkboxes on Page 2 of the Student Application Form for selecting classes.
Save the forms after completing them digitally, or keep your hand-filled copies safe for submission.
STEP 3
Step 3: Submit Your Completed Forms
You can submit your forms using one of the following methods:
Email Submission:
Attach the completed forms to an email and send them to lakesideartistsacademy@gmail.com.
In-Person Submission:
Contact us in advance via email to let us know you’ll be bringing the forms.
Hand in the completed forms on the first day of class. Hand-filled forms are acceptable if you choose this option.
STEP 4
Step 4: Liability Waiver
Upon arrival on the first day of class (January 14, 2025), you’ll be required to sign a Liability Waiver to finalize your registration.
If your child can drive, and you are sending them, you can print off a copy of the waiver and send it with them.
